How It Works

Who’s eligible for the Care Coordination program?

The Cardiovascular Care Coordination Program is designed for individuals who are currently managing or at risk for high blood pressure (hypertension), specifically adults aged 18-85 with blood pressure over 140/90. It's ideal for those who desire personalized support and guidance in managing their heart health and are looking for resources and education to make healthy lifestyle changes. The program also prioritizes accessibility, offering support through local rural health clinics. Essentially, if you're seeking to better manage your blood pressure, partner with a dedicated care team, and lower your risk of heart disease, this program may be right for you.

Personalized Care and Support

We offer personalized care and support through several key features. Each participant receives a tailored care plan designed to address their specific needs and health goals. We also offer regular blood pressure monitoring to track progress and make any necessary adjustments to the care plan. Perhaps most importantly, you'll have access to one-on-one guidance from our dedicated team. This team includes experienced nurses, community health workers, and physicians who are committed to empowering you with the tools and knowledge you need to effectively manage your high blood pressure and improve your overall heart health. They'll be there to answer your questions, provide encouragement, and celebrate your successes every step of the way

Education and Empowerment

We believe that informed patients are empowered patients. That's why we partner with local rural health clinics to provide accessible resources and educational sessions.
