Rural Alabama
Prevention Center (RAPC)

We proudly employ Community Health Workers (CHWs) who play a vital role in implementing program activities across various churches in Greene, Monroe, Escambia, Conecuh, Butler, Marengo, Clarke, and Washington counties. These dedicated individuals are essential in fostering community engagement and ensuring that health resources are accessible to all. In addition to our work with CHWs, we serve as the lead agency responsible for coordinating all community initiatives and funding, which allows us to effectively address the unique needs of each county we serve. Through collaboration and commitment, we strive to enhance the overall health and well-being of our communities.

Located in Tuskegee, Alabama, TAHEC boasts one of the largest educational facilities in the Delta Region. For over a decade, TAHEC has been a vital partner in improving community health within Macon, Barbour, and Bullock counties. TAHEC plays a crucial role in:

  • Expanding access to vital health services: They provide residents with access to diabetes prevention classes and preventive health screenings through a network of community outreach initiatives, including health fairs, churches, schools, and other community venues.

  • Empowering local communities: TAHEC employs dedicated Community Health Workers (CHWs) who work within local churches in Barbour, Macon, and Bullock counties to implement impactful health programs and connect community members with essential resources.

Community Health Resource and Awareness Resource (CHEAR)

The Community Health Education and Awareness Resource (CHEAR) is a respected non-profit organization (501(c)(3)) based in Livingston, Alabama. With a strong reputation for service within Sumter County and across the state, CHEAR is dedicated to improving the health and well-being of its community. CHEAR provides a wide range of essential services to residents, including:

  • Transportation assistance: Ensuring access to healthcare and other vital appointments.

  • Home-based care: Providing in-home support and coordination of services.

  • Diabetes education: Offering valuable classes to manage diabetes effectively.

  • Medication assistance: Helping individuals access and afford necessary medications.

  • COVID-19 testing: Providing convenient access to vital testing services.

  • And many more vital services.

CHEAR employs dedicated Community Health Workers (CHWs) who work within local churches in Choctaw, Pickens, and Sumter counties to implement vital health programs and connect community members with the support they need.

The Alabama Cooperative Extension System (ACES) is a leading outreach and engagement program, a joint initiative of Alabama A&M and Auburn universities. ACES is renowned for its commitment to providing high-quality education and research-based information to diverse audiences across the state. ACES plays a crucial role in:

  • Fostering community partnerships: Collaborating with individuals, groups, and agencies to empower communities and improve lives.

  • Reaching every corner of Alabama: Offering the only statewide delivery model, ensuring access to valuable resources for both rural and urban residents, including underserved communities.

  • Empowering local communities: ACES employs dedicated Community Health Workers (CHWs) who work within local churches in Dallas and Wilcox counties to implement impactful health programs and connect community members with essential resources

Sowing Seeds of Hope is a transformative partnership between the people of Perry County and Alabama Cooperative Baptist Fellowship (CBF)-related churches. This 25-year commitment is dedicated to enhancing the lives of Perry County residents through a wide range of initiatives, including:

  • Home repair and housing solutions: Addressing critical housing needs.

  • Healthcare assistance: Improving access to quality healthcare services.

  • Spiritual renewal opportunities: Fostering spiritual growth and community.

  • Educational resources: Expanding access to educational opportunities.

Through building strong relationships, forging impactful partnerships, and collaborating with like-minded individuals, Sowing Seeds of Hope strives to make Perry County a thriving and vibrant community. Sowing Seeds of Hope employs dedicated Community Health Workers (CHWs) who work within local churches in Perry County to implement vital health programs and connect community members with essential resources.

Established in 1973, Hill Hospital provides essential healthcare services to the Sumter County community, including Emergency Care, Inpatient Care, and a Rural Health Clinic. To address the growing need for comprehensive care, Hill Hospital partnered with RAPC to enhance patient outcomes. This collaboration focuses on providing coordinated care for patients with diabetes and heart disease who require specialized care management.

Rural Health Medical Program, Inc. (RHMP), a Federally Qualified Health Center, has been a cornerstone of healthcare in the Black Belt region of Alabama since 1977. Guided by a mission to provide exceptional, affordable care, RHMP offers a comprehensive range of services, including family medicine, women's health, dental, and behavioral health. RHMP attracts patients from across the state, drawn to its high-quality care and commitment to patient satisfaction.

 

To further enhance patient care and improve outcomes, RHMP clinics partnered with RAPC to implement a robust care coordination program. This collaboration focuses on providing specialized support and guidance to patients with diabetes and heart disease across all RHMP's rural clinic locations.
