Greetings,

Healthcare post-COVID-19 continues to be challenging for the healthcare community. While it has taken time for us to heal and realize normalcy, we are continuing to
address the elephant in the room - ACCESS. Rural communities face numerous challenges in accessing health care, including, among others, facility closures, workforce shortages, and transportation challenges. Access to essential health services, including primary, emergency, prevention, care management, and social and
behavioral health services, is critical to preventing
disease.


​Since the inception of the South-West Health Improvement Initiative (SWAHII), Rural Alabama Prevention Center (RAPC) and its partners have impacted the lives of so many through the implementation of preventive health, education, and care management. As RAPC and its roster of partners continue to work to ensure that residents in rural Alabama know their health risk factors, we encourage everyone to get involved in your health, your family's health, and your community's health. With all of us working together, we can ensure a healthier Alabama.

Our Story

Founded in 2014, the Rural Alabama Prevention Center, Inc. (RAPC) is a 501(c)(3) non-profit organization dedicated to improving the health and well-being of residents in rural West Alabama. Guided by a dedicated Board of Directors, RAPC has secured over $6 million in funding to implement impactful health programs.
 

Our journey began with a focus on chronic disease prevention. We've partnered with the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA) to launch initiatives like the "South-West Alabama Health Improvement Initiative (SWAHII)" and the "Tri-County Health Improvement Special Project (TCHISP)," empowering communities to make healthier choices and prevent diseases like diabetes, heart disease, and hypertension.

RAPC has played a crucial role in community health during the COVID-19 pandemic, leading testing and vaccination efforts across Alabama. We continue to build on our successes, collaborating with leading institutions like the University of Alabama and the Tuskegee University to address critical health needs and reduce health disparities.​

In 2023, Rural Alabama Prevention Center received another round of funding from the Department of Health and Human Services, Health Resources and Services Administration (HRSA), to expand on the success of the Southwest Health Improvement Initiative (SWAHII) by strengthening the Diabetes Empowerment Education Program (DEEP) curriculum in churches with a primary focus on Type 2 diabetes prevention and management, and by reducing hypertension among rural health clinic patients with uncontrolled blood pressure.  

 

​Through strong community partnerships, innovative programs, and the dedication of our team, RAPC is making a lasting difference in the lives of individuals and families across rural Alabama. We are committed to creating healthier communities where everyone has the opportunity to thrive.
