Type 2 Diabetes Prevention and Empowerment

The Type 2 Diabetes Prevention and Empowerment Program provides evidence-based education, self-management support, and community interventions to help individuals with or at risk of type 2 diabetes achieve and maintain healthy blood sugar levels through lifestyle changes like diet and exercise, thus reducing long-term health risks.

EPIPHANY

EPIPHANY (Equity in Prevention and Progression of Hypertension by Addressing barriers to Nutrition and Physical Activity) is a study that aims to improve blood pressure and reduce disparities among African American Alabamians by addressing barriers to nutrition and physical activity.

Cardiovascular

Care Coordination

The Cardiovascular Care Coordination program provides education, self-monitoring tools, and care coordination to at-risk adults with high blood pressure, aiming to reduce uncontrolled blood pressure and lower the risk of cardiovascular disease.

Tri-County

Health Improvement

Special Project

The Tri-County Health Improvement Special Project (TCHISP) was a three-year initiative that used Community Health Workers in local churches to promote healthy lifestyle choices and prevent cardiovascular disease among residents of Greene, Hale, and Sumter counties with no history of heart disease.

COVID-19 Testing and Vaccination

The Greene County COVID-19 Testing and Vaccination Team, with support from multiple funders, expanded its reach throughout Alabama, providing nearly 5,000 people with testing, vaccinations, and educational outreach.
