This project highlights the ongoing work of the Rural Alabama Prevention Center (RAPC) to improve health outcomes across 17 rural counties in Alabama. Through community partnerships, health education, and wellness initiatives, RAPC empowers individuals and families to prevent and manage chronic conditions like diabetes and hypertension. Our team of dedicated community health workers connects residents with resources, screenings, and support to create lasting change.
